Immunization Disclosure Statistics

The Chief Medical Officer of Health has directed all publicly funded school boards in Ontario to implement a COVID-19 immunization disclosure for the 2021-2022 school year. The immunization disclosure is required for all publicly funded school board employees, staff in private schools and licensed child care settings and other individuals frequently in these settings who may have direct contact with students and/or staff. 

School boards are required to publicly post immunization disclosure data. The following attestation summary report for the Simcoe Muskoka Catholic District School Board will be updated on a monthly basis.
