In-School External Agency/Service Provider

Service providers who wish to offer programs and services within our schools, to our students, are invited and required to submit an application for external agency collaboration.  A central board committee, the Collaborative Partnership Committee in accordance with PPM 149 will review each application. Consideration will be given to those programs and services that are supplemental to those being provided by school board staff, and are consistent with the Board’s mission, vision, and values.

The committee will meet on a regular basis to review applications and will communicate decisions to each applicant. 

Upon approval, a Collaboration Agreement will be jointly entered into by the External Agency or Third Party Service provider and the Board.

Application Information

If an External Agency/Service Provider (regulated or non-regulated or third party) wishes to be considered for the provision of program/services within schools of the Simcoe Muskoka Catholic District School Board (SMCDSB), the
SMCDSB Application for External Agency Collaboration (PDF) should be completed in full and submitted along with all required supporting documentation.  Please note: you must first download this pdf form to your desktop and rename it in order to use the fillable form capabilities.
